The Endothelial Glycocalyx and the Microcirculation: Rethinking Intraoperative Fluid Therapy at the cellular level.

Description

Re-evaluate traditional, volume-driven fluid resuscitation strategies by shifting the focus to cellular physiology, microcirculatory perfusion, and the endothelial glycocalyx layer (EGL).

Objectives

• Analyze the role of the endothelial glycocalyx and cellular membranes in maintaining fluid homeostasis and preventing tissue edema. • Differentiate the cellular and microvascular impacts of synthetic crystalloids, balanced salt solutions, and colloids during the intraoperative period. • Identify the cellular consequences of hypervolemia, including mitochondrial dysfunction, endothelial shedding, and impaired tissue oxygenation. • Evaluate real-time macro- and micro-hemodynamic monitoring techniques to guide personalized, goal-directed fluid therapy (GDFT). • Formulate a patient-specific intraoperative fluid plan that minimizes cellular injury and optimizes organ perfusion for high-risk surgical patients.
