Description
This session focused on strengthening primary health care organisation, prevention and continuity of care across the life course. It explored the role of Primary Care Networks (PCNs), community health promoter linkages, bi-directional referral pathways, facility readiness and multidisciplinary approaches in improving coordination and continuity of primary health care services. Presentations also examined the establishment and organisation of PCNs in different county contexts and the integration of specialist and multidisciplinary approaches to PHC delivery. The session further explored prevention and continuity of care across the life course, including comprehensive wellness and early detection of non-communicable diseases, cardiometabolic risk and obesity, frailty and glycaemic control among older adults, transition care for adolescents with chronic illness, and the experiences of families of patients admitted to intensive care. Scientific presentations, a Q&A session and poster presentations provided evidence and practical experiences relevant to strengthening person-centred, integrated and continuous primary health care.
