Conducting staff sensitization and training on sentinel events and near misses. Identifying, documenting, and reporting sentinel events and near-miss incidents. Reviewing incident reports and analysing trends in reported events. Conducting root cause analysis or other appropriate incident investigations. Holding multidisciplinary meetings to discuss incidents and lessons learned. Developing and implementing corrective and preventive action plans.
Objectives
o enhance staff understanding of sentinel events and near misses and their significance in healthcare delivery.
To improve the early identification, documentation, and reporting of sentinel events and near-miss incidents.
To strengthen staff capacity to investigate incidents and identify underlying causes and contributing factors.
To promote a culture of patient safety, transparency, and non-punitive incident reporting.
